Page 2 has a form that has been printed and includes written information (put into parenthesis):
"(Jackson) Council, No. (45) Jr. O. U. A. M. This is to Certify, That (J. H. Rupp), was admitted into this Order on the (31st) day of (December) 189(7) C. (A. R. Downey) R. S."
